EV Challenge High School Application Form - This form should be used if you are unable to register online or if web site instructions direct you. Schools should register with the online registration form.
High School Check Off List - School officials are required to use this check list to ensure they have completed all tasks prior to arriving at check in on Friday morning. 1 form necessary for each school.
High School Registration List - This form should include all participants and visitors from each school. 1 form necessary for each vehicle team.
EV Challenge Release Form (pdf) - This includes the Publicity Release Form that everyone must sign and the Parental Consent and Minor Acknowledgement forms that all minors must have properly completed. 1 each for each individual. Also in MS Word format.
High School Vehicle Data Sheet - A sheet needs to be completed for each vehicle.
School Contact / T-Shirt Request Form - All schools should complete this form to provide information on who to contact to resolve registration issues. Everyone who should be considered for receiving a T-Shirt should also be listed.
